How to claim

When seeking services from a health and medical provider (e.g. doctor, chiropractor, pharmacist, etc.), please present your Coverage Card and valid photo identification (e.g. student ID card, driver’s license). Providers who are set up for direct billing will not charge you for eligible services. However, if you are charged for services, download, print, and submit the claim form, along with your original receipts, to Manitoba Blue Cross and you will be reimbursed for eligible expenses.

You can submit to Manitoba Blue Cross:

PRESCRIPTION DRUGS

Please note that MISHP only provides coverage for prescription drugs listed in the current Manitoba Pharmacare formulary, after an income-based deductible has been reached.

Your deductible is the portion of your prescription drug costs that you’re required to pay before your plan begins to cover them. It is calculated using the information provided on the deductible calculation form.

Download, print, and mail the deductible calculation form to the address on the form (above) to receive the appropriate reimbursement for your prescription drug expenses.

Prescription Drug Special Authorization Request (EDS Form)

When prescribed a drug, please inquire with your physician or pharmacist if it is a Part III Exception drug status (EDS) prior to it being dispensed. Part III Exception drugs require pre-approval from Manitoba Blue Cross or the claim will be rejected. Download and print the prescription drug special authorization request form and bring it to your physician to complete. Once done, mail to the address on the form (above) to apply for the necessary pre-approval.
